Incident to Services

If an established patient sees the Physician Assistant for a chronic problem and med refills, say, hypertension, but while in the office, mentions a new problem that the Physician Assistant has not treated before, say an earache, can the visit be billed as incident to?

The Medicare guidelines aren't specific enough to answer that question with a citation. In my opinion, changes to an existing plan could be billed incident to, but not a new problem at the same visit.
