Developmental Screening 96110 using ASQ form

In the CPT® book it says that it is expected that the administraio of the testing will generate material that will be formulated into a report. The scoring on the last page of the ASQ form is done by clinical staff, the doctor initials it and sometimes notes in the note under Plan 'ASQ within normal limits' and the ASQ is scanned into the EMR. There is no other documentation by the physician. Is this enough documentation?

Medicaid is the only insurance co. we have come across who is particular about the 96110. We contacted out state Medicaid and the only requirements were "Primary care providers must document the screening tool utilized, with interpretation and
report, in the child’s medical record." We have in our templates whether the patient passed or failed the ASQ-3 tool. If they failed, the provider will elaborate and set up a plan. If they passed, that is all that is needed. We also make sure to have the form scanned in the patients chart. We even called our state Medicaid to make sure the information we had was correct. I would contact your state Medicaid to see if they want anything else.
