CPT® 36415

When is it proper to use this code? If an office level is billed also is this supposed to be included in the E/M or billed in addition and restricted to own dx? Or is it to be billed only if it's a nurse only visit and no encounter with a provider?

I bill this administration code anytime a patient is stuck for a lab test.
