Z01.419

I am being told that z12.4 code cannot be used with the z01.419 and yet v76.2 was often used with the v72.31; not only that but sometimes pap smear is done with well woman exam and sometimes not and sometimes the screening is for vaginal and not cervical; where can the rules be found about this?
