Ob - HPT+ and TVUS+ at first visit

If the the reason for an OV with the Ob-Gyn is to confirm pregnancy and the patient comes in with amenorrhea and positive home pregnancy test, and a transvaginal ultrasound confirms positive for pregnancy, does that visit begin the global package, or can that visit be billed as an E/M service?

I'm new at Ob (obviously), but there are 5 Ob-Gyn in our multi-specialty practice, each of whom bill differently, the documentation habits are dissimilar in this clinical scenario, too.

Ob

probably will see multiple opinions on this by payer. If visit was a scheduled OB visit, it will be part of global package. If patient has menses suppression (626.8) and the E/M confirmed pregnancy but OB record not initiated during the encounter,-may allow E/M. V22.0 is coded if the patient leaves the office with confirmed pregnancy.
