remote pt/inr checks

I know with Medicare patients we use code G0250 for the testing of pt/inr at home (4 tests). If the same is done for a private carrier, do we use 85610-qw? This is confusing as some are telling me I should use 99091 however this only allows one in 30 days. Any help is appreciated.
