Doc went to see our pt at her house last year. After evaluating her, pt requested to trim her toenails. So, doc did home visit eval & toenail trimming on that day. We got paid fr HPlus last yr, then this year, they took their money back stating its part of the global package. Can I rebill the claim w/modifier 25? need advice. thanks!
