Unsure of modifier

Our patient was seen in the office for a problem focused appointment to have 2 shave biopsies done and also cryotherapy of actinic keratosis - 13 total. I think the proper coding is 99212-25 11100 11101-51 Dx D48.5; 17000-59 17003 Dx L57.0. I am a little unsure of the 59 modifier. Any input is appreciated!

Why are you billing an E/M?

11000 is for punch biopsy. Look at the codes in the 113XX series for shave.
