A V code can definitely be a primary diagnosis in situation such as Cancer Survellance or Pre op/post op visits, but some insurances if you are in a specialty will not cover routine visits.

There are some V codes that, through payer edit, can't be a primary DX. Medicare doesn't like V45.81 as primary, for example. But there is not a blanket disallowance.

. . . that is as long as you are billing for a 'primary'/physical healthcare issue. Should you happen to be billing for Mental Health, use of a "V-code" in the primaray position will usually mean there is NO reimbursement at all.
