injectable,flowable wound matrix

could you pleae help me with a CPT® code for a debridement and injection
of 3cc of Integra injectable,flowable wound matrix, in OR, in hospital.
I code ASU for the hospital. I see these injections may not be reported
under the 15--- series of codes per the instructions in the book.
The only codes I come up with are 11951 and Q4114 and I'm not
convinced either of these are correct
