Dm w manifestation

I HAVE QUESTION ANY FEEDBACK WOULD BE HELPFUL... A pt comes in to the clinic to be seen for a DM check, the pt has DM neuropathy, HOWEVER the provider doesnt address the manifestation (which is the neuropathy) just addresses the DM which at this visit happens to be controlled. Provider doesnt check or address anything w the manifestation, do you code to 250.00 or to the 250.60 w 357.2? Confused on this because i have read several documents telling me two different things. I believe Each encounter has to stand on its own from the documentation so wo mention of the manifestation wouldnt you code to 250.00

Provider needs to list both. Coding is really incomplete without that detail.
