Optometry Coding-65222, 99213, & 99285

I have a coding question that I can't seem to find the answer to....
I received a claim from one of my Optometrists with the following info:
Diagnosis Codes: 918.1 & 930.0
CPT® Codes: 99213, 65222 & 99285
I am not sure what order to place these CPT's in and whether or not I need to use Modifiers. I know I will need a Modifier that shows which eye (Left or Right) to use for the 65222, but I really need help with the rest.

Why is your provider coding for the Emergency room visit 99285 and an o/f 99213 on the same day? Was he the treating provider when the patient was brought into the ER? if not he/she cannot code for that visit, just the ER doctor that treated him would use that code. if the patient came back to the optometrists for f/u after he left the ER then he can bill for the 99213 w/modifier 25 along w/the 65222 if it substanuates an additonal o/v. if the patient came back to your office just to have the forgein body removed then you cannot code for the o/v just the procedure. also who removed the forgein body, the ER of was it removed when the patient came into the office?
