Procedure code 88305

Medicare rejected my claim for Code 16 - Claim/service lacks information which is needed for adjudication. I bill insurance for a pathologist.
The CPT used was 88305 and the ICD9 211.3. Any suggestions?

check everything

usually when i get that denial code its for something very simple such as wrong date of birth,name spelling,the name not same as what is on medicare card...double check icd9 that there is no other digit also....if all else fails you can "beg" a medicare csr to tell you why it denied...hope that helps
