Extended HPI for routine follow up using 1995 guidelines

Any suggestions examples on an extended HPI using the 95 guidelines for established patients coming in for routine follow up(HTN, DM, hyperlipidemia)etc?

Per CMS, yes you can use the "chronic conditions" logic for 95 guidelines (HPI). Although this information is physically present in the 97’ DG it is also implied in the 95’DG.
Follow documentation guidelines, scoring, etc. for your local carrier/Medicare Administrative Contractor (MAC).Some CMS carriers allow the status of
3 or more chronic conditions as an extended HPI with 1995 exam.
