annuals
For my patients who I only see once a year for their annual, and I have on meds for depression and migraine. Since I review that part of their hx as well - I am always going to bill a 99213 + annual. Is this good practice?

The CPT® book states that if there is significant extra work a problem oriented visit may be billed as well as the preventive.
Be sure the extra work is documented: if the HPI is no complaints, stable on meds, and the plan is renew, there is no extra work, in my opinion. I audit an extra problem oriented visit when:
Patient is symptomatic from a new complaint or existing complaint
HPI/ROS describes symptoms (just as if it were a stand alone visit)
A/P shows treatment or treatment change, diagnostic (not screening ) test and follow up.
Patient will have co-pay/deductible/patient due amount.
