E&M Assignment for ED Facilty

I am new to ED coding for the facility and have a question concerning the E&M assignment for fractures any suggestions would greatly be appreciated.
What level would you assign for fracture of toe-buddy taped-Level 3?
fracture of finger xray and splinted referred to ortho-Level 4?
fracture of femur or humerus-Level ?
Tib or fib or both splinted or aircast refer to ortho-Level 4?

Facility E & M is not based on history, exam, medical decision making. It is based on a point system designed to capture revenue based on utilization of resources. Each facility develops their own point value and calculation for the code level. You need to know how this facility has set up their point system.
