Billing Procedure codes with Modifiers

How are you,

Can Procedure codes 27486, 27506 and 20680 be bill together and if so what are the modifers that should be used. This is for Medicare .

20680 is included in code 27506 and code 27486.
The diagnosis codes must reflect medical necessity for all procedures.
