Billing for transition care CPT® codes 99495 and 99496

Can we bill for the 99495 and 99496 transition codes from the hospital in addition to the office visit or are they specifically replacing the office vists in these cases?

Physicians should note that the first post-discharge face-to-face visit with the patient is part of the TCM service and not reported separately. Additional E/M services may be reported separately.
