what is icd9 code for annual well visit(G0438) for medicare?

1. Please help me with ICD code for Personalized Preventive Physical(Annual Well Visi=AWV) for Medicare patient?
2. If patient is new medicare, can we bill G0101(wellcome to medicare) and annual well visit within a year?

ICD9 code I use is V70.0. G0438 cannot be billed within 12 months of becoming eligible for Medicare. G0402 within the 1st 12 months.
