re: F/U after hernia surgery
The post-op visit for hernia repair is included in global period (no charge). If the physician evaluates and manages the diabetes, it may be billed with appropriate E/M code (99212-99215) as documented for the diabetic condition. You must place modifier -24 on E/M code. A level 4 visit requires a detailed history and physical and moderate complexity MDM. The ICD-9 code would be from category 250.xx. The level of E/M must be documented and medically necessary.
re: F/U after hernia surgery
Modifier 54 is inappropriate for your scenario, and never used on an E/M code.

Thank you...
How about the staple removal?
re: F/U after hernia surgery
Cannot bill separately included in global period.
