denied claim

If a physician's office billed on 3/12/ and then again on 3/20 for the same codes e.i. , 99214-25 and 17110.
Why would 17110 be denied when billed the second time?

17110 has a 10 day global on it. You would need to add -58 onto 17110 for the second date of service.

Thank you, and where will I find the 10 day or any global rules for surgery?

If you have a 2011 CPT® Expert book, the globals are listed with each CPT® code. Otherwise, I find then on my EncoderPro software. I am sure there are websites out there as well to help you find those. If I can come up with any, I will reply again :)

Thank you so much for your time. I have the standard edition and I cannot find global anything in this book. I will keep trying.

I found a website, kind of annoying but once you get used to it, it's quite easy.
www.cms.gov
scroll down and on right hand side under top 10 select #8 physician fee schedule lookup
then next page scroll down to middle of page and click on start search
make sure 2011 is in 1st drop arrow
type of info-payment policy indicator
single HCPCS code
enter the code
drop arrow to select modifer
hit submit
once info shows up, scroll down and 4th column from left is your global period

make sure you save the website in your favorites to make life easier
