thrombectomy and initiation of thrombolysis

What is the correct way to code for thrombectomy and initiation of thrombolysis following an upper extremity angiography. 2 seperate relook angiograms were done the following day post thrombolysis. please help

Refer to the CPT® Guide Forward to the Transcatheter Procedure section where codes 37211-37214 are located.

37211 & 37212 are for the 1st day of treatment (and last if it all takes place on the same day).

37213 is used for subsequent days that are not the 1st or the last day of treatment.

37214 is for the final day of treatment.

Report any of these codes only ONCE per date of treatment.

If you performed a - medically necessary - diagnostic angiogram of the upper extremity on the same day as the intervention, you will need to modify the 7XXXX code with a -59 modifier.
