I am wondering if anyone knows if a primary care doc should still bill a 93000 if they are performing the EKG, interpreting but then also sending to cardiologist for review or should we be billing the 93005 only

Who is documenting the interpretation and report? Only one physician will be paid for that. If the cardiologist is doing it, then you should bill only for the technical component, 93005.
No modifiers because these codes are already broken down into technical and professional by CPT® definition.
