Medicaid Preventive Visit Denials

Hi,

Medicaid NY denies most of our preventive visit codes with the denial reason CO96 (Non-covered charge(s)) eventhough we bill it with V202 or Z00129 codes. And we are not able to get an appropriate reason from them. Can anyone explain a general reason why Medicaid denies preventive visit codes??
