please respond soon regarding how to bill 58120, 76830 and 64435

Can any body suggest how to bill these codes with modifiers?
DX 1. 666.24 2. 626.8
CPT® 1. 58120 2. 76830 3. 64435

try billing like i have it below, not sure if the DX's are w/the appropiate CPT® codes, but i think this should work, let me know:)



58120=626.8
64435-59=666.24
76830=666.24

go to www.cms.gov/Medicare/Coding/NationalCorrectCodInitEd/NCCI-Coding-Edits.html

Scroll to "Related Links" and choose Physician coders or Hospital (facility coders) there will be a download filter this for the codes you want to see if they are bundled.

I looked under physicians- and it looks like two of your codes are bundled w/no modifier exception
6th column: 0 = there are no modifiers associated w/ NCCI that are allowed to be used w/ this code pair. There are no circumstances in which both procedures of the code pair should be paid for the same beneficiary on the same day by the same provider. 1 = modifiers associated with NCCI are allowed with this pair when appropriate 9 = NCCI edit does not apply to this code pair.

I have one more question .
Do I need to add modifier to 76830 and what modifier i need to add it?
