Multiple Ventral Hernia Repair

The physician creates a midline incision and starts to repair this massive ventral hernia but during the repair he encounters additional ventral hernias which cause him to extend his original incision. He uses one large mesh to close this repair. Do we code once 49560 or do we code 49560 49560-59 for the additional hernia repairs?

Same incision I believe would only be one hernia code but you could use the 22 modifier for the increased work and of course 49568 for mesh.
