Credentialing at different practices?

I am a physician who will be working part time at two different medical practices. How do I apply to insurances so that my claims at each of the practices gets to the right place?

I don't understand how a claim get processed and tracked, in terms of TIN and NPI... I have my own SS# and NPI, I also made myself an LLC and have my own TIN for the LLC and entity NPI, and of course each practice I'm joining has their own TIN and NPI.

This might depend on how your contract with the practice is laid out. Typically, a credentialling person in the medical practice will add your NPI and/or provider number to the TIN of the medical practice, presuming you are working for the organization, on each contract they hold with payers.
