Pulse ox

A pulse ox was done on 10/18/10. Nurse demonstrate the use of the machine. Pt came back the next day but no reading was recorded. The physician ordered a repeat one. Do I need to use modifier 76 of 77 for 94760? And can I bill for 74664 twice? Thank you.

Most Payors consider the pulse ox not billable seperatly that it's included in the E&M

The first visit (10/18/2010): 99211-25 and 94762

The second one (10/26/2010): how do I bill it? A nurse visit with a repeat overnite pulse ox. Will it be 99211-25 and 94762. Do I need to put a modifier? and it was ordered by a diff physician.
