For HEENT does the doctor have to mention head, eyes,ears,nose and throat?

Can I give credit for this HEENT: The pupils are equally round and reactive to light and accommodation. Exraocular muscles intact. Oral mucosa is moist?

Also Extremities: No edema. Does this count for extremities? Or is no edema part of Cardovascular?
