I have a denial for 99396

Per pan-american, 99396 is not included in their indemnity plan. Can I change this to 99215 and appeal? The patient is an established patient.

If the patient received a well exam, and it is documented as as such, then changing the code for the sole purpose of getting it paid would be *fraud*.

99215 and 99396 are both E/M visits. Please explain to me why it will be fraud and from what I see both have two of the three components needed and if it so, what you use to appeal the claim?

I would submit a corrected claim using 99215. You probably don't need to do an appeal at this point, just a rebill.
