When providers bill 90935, are they required to document in the medical record their physical presence at some time during the dialysis or does the record just have to show that they ordered it? Thanks.

To be paid for CPT-4 code 90935 the physician must be physically present with the patient during the dialysis procedure, and the medical record must document this.

Thanks. Would you happen to know the Medicare (Noridian is our local carrier) chapter and section where this is stated. Our physicians will want to see it in black and white.

You can see it includes evaluation and management, thus, physician presence
