ulcer debridement

When billing 11040-11043, what is the proper way to bill for more than one unit on the same limb? I bill for podiatrist offices and they often debride more than one ulcer on each foot. I have had success billing two, using one unit on each claim line with RT/LT mods and a -59 mod on the second line; however, when I bill more than one code (same limb), I am at a loss as to how to get more than one of them paid. I always seem to end up with duplicate denials on all but the first line. (I am speaking strictly of Medicare billing here, btw.)

If it's multiple ulcers on the same limb we usually just put that as units. like 11041RT 3 units for 3 ulcers then of course the 59 on the second line for the other limb like you said.
