Billing for Aphakic Contact Lens to patients in SNF

We have a patient in a skilled nursing facility that wears an aphakic contac lens. How do we bill for the lens and who do we bill. We have billed DMERC carrier and received denial and we've billed the SNF resulting in no payment. Who do we bill, how? (modifiers and place of service)?
