inpt consults vs oupt consults

Hello-

Is there any difference when coding the two? For example, do they have to have all 3 elements in the actual note to be qualified for a level? Do they still have to have the "request" stated? They still need the PFSH? Is there any "blanket statements" allowed? For example Dr. X sees the patient and wants a consult from Drs. A, B, C do the Drs. A, B, C have to state in the notes that Dr. X was requesting this or can we go by Dr. X's note that states he wants Dr. C's opinion?

Consults

Consultations, inpatient or outpatient require documentation of the 3 key components. The requesting physician must document the request in his/her notes and the consultant must document request in his/her note.
They need the PFSH.
REQUEST from another physician for consultant’s opinion must be clearly documented in BOTH the receiving and referring physician charts.
Referring MDs must have it in their plan of care. The REASON for the consult must be clearly documented in the medical record. Opinion RENDERED by the consultant with RECOMMENDATIONS for treatment. REPORT goes back to the referring/requesting provider.
