outpatient surgery

pateint presents to outpatient surgery department with a suspicious skin lesion involving the upper trunk. lesion measures 1.8cm and the margin required to adequately excise the lesion includes 0.2 from both sides. pathology is negative for cancer noting actinic keratosis assing the appropriate icd-9 diagnosis codes(s) and CPT® code(s) for the surgeon. please help me code this
