Palliative Care ICD9 codes

Do any of you have a denial issue when using a primary dx (that is causing the pain of patient) and V66.7 when a patient receives concurrent care and the primary provider is using the same principal dx? Thank you.

I believe that the Coding Clinic guidance is to use V66.7 as a secondary code and use the principal diagnosis as the primary code.
