Question for Everyone Here

Hi all,
I am hoping you assist me with some questions our office is having by letting me know how your office does this...
The Dr being the caring person she is, has always offered "free" care to all her employees, mainly because she can't afford to offer us all healthcare. This has ranged throughout the years to cover spouses, children, grandchildren, etc. Now she is wanting to define this to only cover employees, and sometimes others. She hasn't made up her mind.
Being the biller, I'm ready to put it all in writting so there is no more questions about who is covered for what.
My question to all of you is this, what kinds of benefits in this area does the practice you work with have? If the dr provides "free medical" how do you bill or not bill that? Do you have this as a write off, or does the dr "pay" and you report this as income? Are their any restrictions on what is allowed or not allowed?

I hate to say it

But, I think you have to talk to your healthcare attorney and accountant.

Wish I could help, but it's above my pay grade.

Employee "free" medical care

Years back, the office I worked for did this "perk" for employees. There were issues with in the office with staff, which we resolved (nosy neighbors, etc)

#1) The office manager or the Coding Supervisor were the only ones authorized to enter the charges into the system due to HIPAA (dx codes, etc)

#2) The charge was entered with the appropriate level of service, but the dx code used in the system was NODX (if your system does not support that, use a generic no dx code)

#3) The charge was then adjusted off the account. We used a specific adjustment code to do this.

The superbills were destroyed once they were entered.

We also at a later date entered the code (appropriate level) with a zero charge amount.

You want to keep track of the services being rendered to the patient, but you need to protect their diagnosis codes.
