99354 Add on billed with E&M

Does the E&M code need the modifier 25 to bill the add on code 99354? Some carrier deny without the modifier 25 on the E&M and others deny if billed with the 25 on the E&M. Carriers!!! UHG!

This can sometimes be a tough code to get paid. It is an add on code so my understanding is that a modifier 25 is unnecessary. Just make sure your documentation is right on and that you can prove medical necessity. Our specialty practice does receive reimbursement, but we usually have to fight for it case by case.
