decision for surgery

Here's the scenario. Patient comes to see us and needs to have a surgery performed. He doesn't want to schedule it now for whatever reason. The doctor says call us when you are ready. The patient calls back a month, or two, or three months later and has now decided that he wants the procedure done. We need to see the patient to update his H & P for the surgery facility because we are now outside the 30 day period of their requirement. The patient then returns to the office for an updated H & P. The same except when the patient is here for that initial visit, they want to schedule the surgery in 6 weeks, or "after I am done harvesting", or whatever. The surgery is scheduled but again is outside the 30 day time frame. the patient must come back within the 30 days prior to surgery for an updated H & P.

the physicians think I should charge the patient a 99212 for this return visit. I think it should be a no charge. I need written proof of how this should be done so I can show the physicians. Please help. Thank you.
