Incident-To Rules - State of Missouri

Does anyone know the rules or where I can find the rules for the situations below? Any information would be greatly appreciated. I have tried to contact the Missouri State Board of Nursing, but have not received any information.


1. When the physician is in the suite and the patient is seen by an NP, Medicare charges and managed care charges can be billed under the physician.
2. When the physician is out of the suite, but within 30 miles of the suite, and the patient is seen by an NP, Medicare charges should be billed under the NP’s Medicare number and managed care charges should be billed under the physician.
3. When the physician is out of the suite and is more than 30 miles from the suite, the collaborative agreement between each NP and the physician assigns another physician a the designated supervising physician, so the NP’s should be able to bill under their own Medicare numbers, and managed care charges should be billed under the collaborating (original) physician.
