S.C.A.R.E.D. - Screen for Child Anxiety Related Disorder and Mood Assessment Checklist are being used by our Pediatric providers to determine whether or not a child has anxiety. These are 1-2 page form; questions answered by the children. Does anyone else use these forms in their practice and what CPT codes are you using? Thank you.
