62290/72295 in 2012

I have a Workers compensation Case where I received a denial for CPT® code 72295 stating that it was included in the amount paid for CPT® code 62290 for an ASC. I however disagree, and they also did not pay per the fee schedule based on te Principal diagnosis code I used of 81.92. Any suggestions?
