Repairs in same anatomical area with 2 different diagnoses

The provider performed an excision and repair on each arm and each site had a different diagnoses. Medicare is denying stating only one repair can be billed regardless of the diagnoses. Has anyone seen this before and/or should we add the -rt and -lt modifier? Thanks
