CPT® to RVU on

If I do a surgery (e.g. Free flap breast reconstruction) I get an assigned number of RVUs based on the CPT® code that I submit to my biller. If I do a second breast reconstruction on the same case (bilateral) I only get half the RVUs I get for the first case. Trying to find out if this is a national phenomenon represented in the MGMA numbers.
