CPT® s2083

We have a general surgeon that has recently just started doing the Lap Band Surgery. We are trying to get prepared now on how to properly submit claims for any lap band adjustments that will need to be done outside the global surgery package. This surgery is only going to be done on patients with commercial insurance or self pay. Can you tell me first of all what the normal charge is for this procedure and how to properly submit the claim? Is there anything additional that needs to be put on the claim? Thanks in Advance!
