Female physical exams with PAP and breast exam

What is the correct way to bill for a physical including a pap and breast exam. I know to use the correct age appropriate E & M preventative codes but what about the pap collection and pelvic exam and also a breast exam?

If the pap and pelvic exam are not done on the day of the physical, when the patient returns to have this done can you bill an E & M code with it?

Non-Medicare--the Pap and pelvic are included in codes 99381-99397-cannot bill separately even if done on another day after the preventive exam.
