synvisc injections

Any suggections on how to bill for synvisc injections?? We have been using a inj/drain code but I'm not confident that this is correct....help....

Billing Synvisc

Dx: 715.96 or 715.91 or 715.16 or 715.11 ... Medicare only pays for Synvisc in the shoulder or knee.

Billing : 20610-LT
20610-RT
or 20610-50 and use 1 unit and double the fee.

In some Medicare jurisdictions, it is required that in shots 2-5 (all shots after the first shot) an EJ modifier is to be put on the claim to indicate that the patient is having a procedure that is part of a series. That is not true for all jurisdictions, however.
