Hpi

Our physicians see workers comp patients and they have a nurse or MA take the HPI and then right under the HPI they document the exam and medical dicision making. The physician does not rewrite the HPI or reference to it. Is this acceptable?

Coding guidelines require the billing provider to perform the HPI.
