lap lysis of adhesions with salpingectomy

Patient is status post inguinal hernia repair with chronic groin pain. During laparoscopy the surgeon discovers chronic adhesions in the omentum and underlying inflammation can be felt around to the mesh, which is not visible. Ovary and fallopian tube are not involved. However, as the surgeon dissects the omental adhesions, the fallopian tube begins to bleed persistently. Therefore the remnant of the tube (post hysterectomy)is removed using the Ligasure device. No attempt is made to remove the mesh.
Does CPT® 44180 cover this with Dx 789.041 and 568.0? Or do we need to report Dx 998.2 and change CPT® to 58661? Since 44180 is a separate procedure, we cannot bill for them together.

About Dx 998.2. Can a surgeon bill for repair of intestine in addition to a major surgery code, if he caused the tear himself?
