Myocardial perfusion imaging single or multiple(spect)

If the report has only the SPECT (78464) code in the description and no other codes, like the ejection fraction or wall motion, but the report itself states the add on codes were done. Do I attach the add on codes as well?

If the add-on procedures were documented and medically necessary, then they should be coded as well. Verify with physician.
